1. Team Uniform Order Form

Each team will need to designate a uniform manager.  Orders may only be made by one person per team.

The order form includes a tab for Men’s orders, Women’s Orders and Embroidery information (ONLY for bags, training jackets and pants).  DO NOT have the players names embroidered on bags or training tops/pants, "Loudoun" and/or #s only.  Each team will send their orders directly to Eurosport.  Morgan Kelso is our Europsort Account Manager and he can be reached at 800-967-8326 ext. 1401 or email mkelso@sportsendeavors.com.  Please read the information sheet from Eurosport for ordering and payment information.

Sample sizes are available in the office to be tried on.  The samples may not leave the office. 

Practice t-shirts are available in the Loudoun Soccer office for $9.00 each.
