Sample Game Report Format

Subject: Game nnnn Score (supply the SFL game number in the subject line)
FINAL GAME SCORE (provide the following information)
Team Age Group (e.g. U14 Girls, U16 Boys...)
Home Team Name and Goals Scored
Visiting Team Name and Goals Scored
Game Date, Time and Field Site
RED CARDS OR YELLOW CARDS
Note the player names and numbers who received cards and briefly describe the offense. Failure to report red cards received by your team will result in additional red card points being assigned to your team.
ROSTER EXCHANGE (optional)
Did both coaches exchange rosters prior to the start of the game? If the opposing team had players that were over the age limit for your age group, did the other coach provide the required SFL waiver documentation?
REFEREES (optional)
Were there three referees at the game (Center Referee and two Assistant Referees)? Did the referees do a particularly good or bad job? Feel free to include praise or complaints in the report.
FIELD CONDITIONS (if warranted)
Was the field properly lined, did it have corner flags and were the goal nets in good shape? Were the field conditions safe for play? Keep in mind that many fields have turf that is considerably less than perfect, so please limit any complaints to cases where the field is particularly worse than usual.
MISCONDUCT INCIDENTS
Were there any cases of player, coach or spectator misconduct that may not have been observed or addressed by the referee but are particularly noteworthy? Please do not report every little thing that happens, but if there is something serious that you believe warrants disciplinary action by a club, be sure to note it in your report. Please include a complete accounting of the incident and include the names of the offending individuals. Also include the names of any witnesses who could be contacted to obtain more information.
GAME PROTESTS
If you intend to protest the outcome of a game, you can say so in the game report but do not describe the protest in that report. Instead, send a separate message to
SFLmanager@loudounsoccer.com to provide the details of the game and the grounds for the protest. All protests need to be submitted to the SFL through the Loudoun Soccer SFL Representative, not from coaches, so you should not provide any protest information in your game report.
