Mail-In Registration Forms
The mail-in registration forms for each of Loudoun Soccer’s recreational programs are listed below in MS Word and PDF formats. Just click on a form link to display a form and then print it out. If you save MS Word forms on your PC you can type in the information on the form before you print it and mail it to Loudoun Soccer (you must have the MS Word software to edit the form).
 
Please note that players for the Recreational and Mini Soccer programs can be registered faster if you use Loudoun Soccer’s on-line registration system.
Mini Soccer (Players born between 8/1/03 and 9/5/05)
Recreational (including Challenge) (Players born between 8/1/96 and 7/31/03)
